X-Ray Upload Assistant
If you are having problems uploading your x-rays, please follow these instructions.
Open this page in a separate window to help you through the ordering process
STEP 1: (Patient's Details)
1. You Need to Enter the Patient's Name. (First Name & Last Name)
2. Then, add the Patient Date of Birth, Date of Accident, Date of X-Rays. (NOTE: Please do not confuse the Date of Patient Birth, or the Date of Accident or the Date of the Xray. )
3. Then, select Patient Gender. (Male or Female)
4. Lastly, enter Patient Weight. (NOTE: Please use pounds(lbs.) only)
1. You Need to Input the Referring Doctor's Name. (First Name & Last Name)
2. Next, enter the Email of the Referring Doctor.
STEP 2: (Referring Doctor's Details)
STEP 3: (X-Ray Option)
1. You Need to select which X-ray you'll be uploading. (Cervical or Lumbar)
1. You need to provide your payment details for the X-Ray such as:
- Credit Card Data. (VISA or Mastercard works!)
- Your Billing Address that is associated with the card.
* NOTE: You can only submit 1 order for Cervical and 1 for Lumbar.
STEP 4: (Credit Card Details)
1. To Upload Flexion X-Ray, you can either click Browse Files to select your file or you can just Drag and Drop the file into the white box.
2. To Upload Extension X-Ray, you can either click Browse Files to select your file or you can just Drag and Drop the file into the white box.
3. Once everything is done, you can click the button:
+ SUBMIT: This button will submit your DATA to us. And you may expect an email as a receipt of your submitted DATA.
+ SAVE: This button can make sure that you wont be losing all your inputted data. You can also use this SAVE button to save your Data on your next submission of X-Ray.
